Referral Submission: You have submitted the referral.

Service Agreement Dispatch: A Service Agreement is sent to the Nominee. (1 -2 working days from referral submission)

Client Communication: Contact with the Client or their Next of kin to book an appointment is initiated. (1 -2 working days from referral submission)

Scheduling of Nursing Assessment: A Nurse will be assigned, and a meeting will be scheduled to conduct an assessment.

Assessment Procedure: The assessment is attended to as requested.

Report Generation: A detailed report will be compiled and made available after the completion of the assessment.

Submission of Report: The report, inclusive of recommendations and quotations, will be dispatched to the referring Coordinator. Any additional information for reports should be requested through the Referrer. (5 -7 working days from the date of the assessment appointment)

Adjustments: Should there be any necessity for the incorporation of additional information, adjustments can be made.
